Job summary

Bridgeway Community Health is seeking a Peer Navigator to support our Bridgeway Community Connect team. You will be the first contact for new clients, building supportive relationships and helping enroll in services with a primary counselor.

You will advocate, educate the community about addiction resources, and collaborate with the treatment team to connect clients to services and resources, including 12 Step groups and health services.

Qualifications

• Certified Recovery Mentor (CRM) with MHACBO and PSS-AA certifications required.
• Personal experience with addiction and recovery processes.
• Experience in assessing and diagnosing substance use disorders preferred.
• Knowledge of addiction recovery supports and criminal-justice transitional services.

Responsibilities

• Transport and escort clients to treatment appointments.
• Coaching on recovery practices and pro-social behaviors.
• Assist clients in securing community resources and support.
• Connect clients to 12 Step meetings and medical/dental services; document progress with treatment team.
• Facilitate mentor support groups.
• Coordinate with team and partners to develop treatment plans and discharge planning.
• Coordinate appropriate case management referrals.

Education

Certified Recovery Mentor (CRM) with MHACBO AND a Peer Support Specialist-Adult Addiction (PSS-AA) Certification with The Oregon Health Authority (OHA)
