Job summary

Community Bridges, Inc. seeks a Continuing Care Peer with lived recovery experience to provide outreach, support, navigation, and care coordination for individuals facing behavioral health challenges, homelessness, or crisis.

This role facilitates peer support and resource linkage both on-site and in the community, helping with deflection, sobering services, and transitions to appropriate care levels. The position emphasizes follow-up, advocacy, and continuity of services, with opportunities for

Qualifications

High school diploma or GED required.
Minimum six months of recovery from substance use and/or mental health challenges.
Experience in outreach, peer support, homeless services, or behavioral health preferred.
Driver’s license (valid and in good standing) required.
Clear 39-month motor vehicle record required.
Peer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.

Responsibilities

Provide community-based outreach, support, navigation, and care coordination for individuals with behavioral health challenges, substance use, homelessness, or crisis situations.
Offer peer support, outreach, and resource linkage onsite and in the community.
Support individuals during deflection encounters, sobering services, and transitions to appropriate levels of care.
Ensure follow-up, advocacy, and continuity of services.
