Job summary

VIP Community Services seeks a Peer Support Worker to use lived experience to outreach to individuals not yet in care, support engagement, and re-engage at-risk clients.

The role helps ensure next-day clinic follow-up after mobile initiation, assists in removing barriers to care such as transportation and Medicaid enrollment, and documents outreach in the electronic health record; reports to the Project Director / Licensed Mental Health Practitioner.

Qualifications

• CRPA certification at hire, or within Year 1 with VIP support.
• Lived experience of recovery with empathy and professionalism.
• Experience with MOUD or opioid use disorder populations preferred.
• Bilingual English/Spanish preferred; flexible schedule including early mornings.

Responsibilities

• Conduct outreach and engagement in the catchment area to reach individuals not yet in care.
• Provide lived-experience support and actively re-engage individuals at risk of disengagement.
• Ensure next-day clinic follow-up after mobile initiation and help resolve barriers such as transportation and Medicaid enrollment.
• Accompany and support individuals in navigating clinic-based and community services.
• Maintain documentation of outreach and follow-up contacts in the electronic health record.
• Reports to Project Director / Licensed Mental Health Practitioner
