Job summary

Family Tree Relief Nursery seeks a Peer Recovery Mentor to support families on their recovery journey, providing guidance, resources, and nonjudgmental support. The role involves coordinating transportation, attending appointments, and connecting families with community services.

Responsibilities include engaging up to 20 families, documenting activities, and participating in multi-agency meetings. Requires recovery experience and certification readiness, with confidentiality and HIPAA adherence.

Qualifications

• Associate or bachelor’s degree in addiction studies, human services, family studies, or related field.
• Certified Peer Recovery Mentor certification or the ability to obtain within 180 days.
• Two years of professional experience in a social service organization with recovery services.
• Reliable transportation, a valid driver’s license, and a driving record permitting agency coverage.
• Must have training or record in mandatory reporting and maintain confidentiality per HIPAA policies.
• Two years of recovery from substance use or misuse.

Responsibilities

• Engage with referred families, up to a caseload of 20 families.
• Accept and initiate contact within 24 hours for all assignments.
• Attend and participate in multi-agency and community meetings as required.
• Complete documentation and manage family files per policies and reporting requirements.
• Follow the agency’s service delivery model focusing on timeframes, trauma-informed, and strength-based approaches.
• Provide outreach services from home visits to community support.
• Work with families to set goals through regular in-person contact across wellness domains.
