Job summary

Crossroads Inc. is seeking a Peer Mentor to support the Clinical Lead and Clinical Coordinator in client services within a fast-paced behavioral health setting in Arizona.

You will assist with intake, admissions, case management, and coordination of care for clients in recovery. Qualifications include at least a high school diploma or GED, 6+ months recovery experience, CPR/First Aid/Narcan, TB tests, and a valid Fingerprint Clearance Card.

Qualifications

• Must meet Arizona Department of Health Services personnel requirements: 21+ years old or licensed/certified.
• Recovery experience of 6+ months required.
• CPR/First Aid/Narcan training current.
• TB test current annually.
• Fingerprint Clearance Card valid.
• Clean MVR; background checks required.

Responsibilities

• Assist with intake, orientation, client file creation and maintenance.
• Facilitate Right Track Program curriculum and manage group capacity.
• Maintain caseload accuracy as directed by supervisor.
• Provide crisis management and case coordination across care teams.
• Assist clients with government assistance applications and referrals.
• Attend clinical staff meetings and trainings.
• Perform other duties as required.
