Job summary

Crossroads Inc in Phoenix, AZ is seeking a Case Manager to support the Clinical Coordinator in delivering client services, including treatment planning, caseload management, and discharge planning in a fast-paced behavioral health setting.

The role requires 1-2 years of related experience, a high school diploma or GED, and current CPR/First Aid/Narcan training, with fingerprint clearance and TB testing. It is a full-time, on-site position with travel between sites and competitive benefits.

Qualifications

• Minimum of High School diploma or GED.
• 1-2 years related experience and/or training or equivalent combination.
• Current CPR/First Aid/Narcan training.
• Current and annual negative TB Test.
• Current and valid Fingerprint Clearance Card.
• Clean Motor Vehicle Record (MVR) check performed annually and upon hire.
• Successfully pass a background check and an ADHS check.

Responsibilities

• Develops and provides individual treatment plans; monitors, evaluates and documents client's progress.
• Maintains a client caseload, set by manager, completes all required documentation in a timely manner, including progress notes, evaluations, treatment plans, coordination of care, and discharge planning.
• Provides individual and group intervention to include crisis management as required.
• Facilitates coordination of care to include case review, weekly progress updates in treatment and coordination of aftercare services.
• Participates in clinical staff meetings, trainings and other activities as required.
• Discusses issues regarding program requirements and/or conflicts with clients.
• Assist clients in completing applications for government assistance, probation/court obligations, and referrals to independent agencies.
• Adhere to internal policies and procedures and behavioral health licensure, local, state, and federal rules and regulations.
• Performs other duties, as required.
