Job summary

WellLife Network in Great Neck, NY is seeking a dedicated PROS clinician to provide recovery oriented services, including individual and group counseling, and care coordination for program participants.

You will conduct assessments, develop individualized recovery plans, and maintain progress documentation while collaborating with a multidisciplinary team. This role emphasizes person-centered care within behavioral health and community programs.

Qualifications

• Master's Degree in Social Work, Mental Health Counseling or Rehabilitation.
• Experience with individuals with mental health conditions in individual and group settings.
• Bachelor's Degree in Social Work, Mental Health Counseling or related human service area with 2-3 years of mental health/human services experience.

Responsibilities

• Provide Personalized Recovery Oriented Services including counseling and case management.
• Coordinate treatment/recovery team meetings and outreach as needed.
• Develop and implement PROS classes and activities within PROS categories.
• Maintain documentation of client participation and progress and ensure HIPAA compliance.

Education

Master's Degree in Social Work

Bachelor's Degree in Social Work or related field with 2-3 years experience
