Job summary

RiseWell Community Services is seeking a compassionate Full-Time Intake Coordinator to join our PROS program in Copiague, NY. The role includes screening and intakes, assessing needs, and developing recovery plan recommendations for new members.

The ideal candidate is a Licensed Master Social Worker with experience in recovery services, holds a valid NYS driver’s license, and can coordinate with referral sources while maintaining proper records.

Qualifications

• Licensed Master Social Worker (MSW) required.
• Experience with individuals in recovery from Mental Illness preferred.
• Valid and clean NYS Driver's License; background checks including fingerprinting required.

Responsibilities

• Screen and conduct intakes for the PROS Program.
• Assess needs and develop recommendations for the Individual Recovery Plan.
• Conduct new member orientation and coordinate with referral sources and collaterals.
• Maintain records according to program regulations.
