Job summary

RiseWell Community Services in Copiague, NY is seeking a compassionate Full-Time Intake Coordinator for the PROS Program. You will screen, conduct intakes, and assess needs to inform the Individual Recovery Plan.

You will coordinate orientations, liaise with referral sources, and maintain compliant records, supporting individuals in recovery through personalized services. Strong communication with treatment teams, documentation accuracy, and adherence to NYS guidelines are required to contribute

Qualifications

• Must be Licensed Master Social Worker (LMSW) in NY.
• Experience working with individuals in recovery.
• Valid NYS Driver's License; fingerprinting and background clearances.

Responsibilities

• Screen and conduct intakes for PROS program admissions.
• Perform assessments to determine needs and develop recommendations for the Individual Recovery Plan.
• Conduct new member orientations and coordinate with referral sources and collaterals.
• Maintain records according to program regulations.
