CareStar, Inc. is seeking a Recovery Manager to support individuals across multiple Ohio counties.

This role focuses on assessing needs, creating personalized care plans, and connecting clients with essential services in a remote-capable setting with periodic field visits. The ideal candidate will have a bachelor’s degree in a related field, 3+ years in home and community-based services, and knowledge of Medicaid and behavioral health resources.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field.
• LSW or RN preferred.
• Minimum 3 years of experience in home and community-based services (within last 5 years).
• Experience with individuals with chronic conditions or SMI.
• Strong knowledge of Medicaid and local community resources.

Responsabilités

• Conduct face-to-face eligibility evaluations and assessments.
• Develop and maintain individualized, person-centered care plans.
• Coordinate and authorize services across behavioral health, medical, and community systems.
• Assist individuals in accessing Medicaid and essential services.
• Monitor service delivery and ensure program compliance.
• Serve as lead advocate and care coordinator for each participant.
• Adhere to CareStar policies and procedures.
• Maintain patient information confidentiality and comply with privacy rules.
• Follow the Acceptable Use Policy for CareStar information systems.
