Qualifications

  • Bachelor's degree preferred or 3 years’ lived or professional experience.
  • Ability to obtain CADC/CARS certification within the first two years in the field.
  • Must pass DCFS background check.
  • Valid driver’s license and car insurance.
  • Proficient in Microsoft Office suite.
  • Experience in group facilitation.
  • Additional licensure may be required by contract obligations.
  • Experience with individuals with SUD and recovery pathways.

Responsibilities

  • Provide comprehensive case management and coordinate behavioral health services with public agencies and community resources.
  • Make contact with referred clients within 48 hours and conduct mobile assessments.
  • Encourage and support JCAP assessments for parents.
  • Enroll clients in programs and document progress.
  • Attend juvenile court dates and provide testimony on recovery status and progress.

Division Overview:

Youth & Family Services (YFS) help parents and young people strive to become healthy and restore their relationships. This Division works with partnerships with Human Services, Child Welfare Systems, Juvenile Courts and Community-Based service providers so that together we can help individuals and families obtain the services and skills needed to live positive and productive lives.

We are currently looking for full-time – Recovery Liaison
Starting at $50,000 - $52,000; contingent upon experience, education, etc.

Position Summary:

TASC-JCAP Recovery Liaison will provide comprehensive and specialized case management services for adults with identified Behavioral Health needs referred to TASC from the Juvenile Court System. The JCAP Recovery Liaison will be the initial point of contact for clients post initial assessment. This staff person will provide direct services and make the initial treatment referral based on identified needs of the individual.

Essential Duties and Responsibilities:
  • Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources
  • Make contact with the referred client within 48 hours of notification and will provide mobile assessment service in the home, office or field,
  • Contact parents to encourage parents to complete the Juvenile Court Assessment Program (JCAP) assessment.
  • Contact , locate and re-engage, individuals into SUD treatment services
  • Complete intake packets for program enrollment and participation
  • Attend client juvenile court dates and provide written and oral testimony on the client’s recovery status and treatment progress. Provide transportation to court ,supportive services, agencies and resources
  • Provide referrals and crisis intervention for the individuals’ identified needs
  • Interview individuals, review records, and confer with other professionals to review individual progress, and to determine their suitability for program participation
  • Knowledge of DSM V and ASAM criteria
  • Collaborate with internal and external partners, participate in staff meetings and monthly supervisions, and participate in integrated staffing with individuals receiving TASC services
  • Identify and refer individuals with behavioral health needs
  • Complete and maintain all required individual and program related documentation which includes, documenting the results of screenings, enrollments, service plans, reports, progress notes, discharge summaries and other individual-related activities including accurate reporting of services
  • Monitor TASC database system and contractual/external database system to ensure individual enrollment
  • Identify new potential partnerships by participate in community meetings, trainings and projects including multi-disciplinary staffing, vocational- and educational-programming, institutional counseling, correctional-industries, employers and other meetings assigned
Our Ideal candidate:
  • Bachelor’s degree preferred and/or three years’ lived or professional experience
  • Ability to obtain CADC/CARS certification within the first two years in the field
  • Must be able to pass DCFS background check
  • Must possess a valid driver’s license and car insurance
  • Demonstrated proficiency in the Microsoft Office suite
  • Demonstrated experience in group facilitation
  • Certain programs may require a higher level of education, certification and/or licensures based on contractual obligations
  • Experience working with individuals with SUD to care for self and family members
  • Must understand and respect individual’s pathway to recovery
  • Knowledge of community support resources, organizations, governmental agencies and nonprofits
