Qualifications

• Bachelor’s degree required; master’s preferred in a related human services field.
• CRC credential with master’s degree preferred.
• CASAC; LMSW; LMHC; LCSW preferred.
• Two years of vocational or SUD program experience with patients.

Responsabilités

• Develop and sustain employer partnerships in healthcare, hospitality, construction, green jobs, and logistics in Bronx zones.
• Provide employer education on fair chance hiring and recovery-friendly practices and incentives like WOTC.
• Represent Montefiore in community and workforce meetings to expand the employer network.
• Coordinate with counselors to place job-ready participants with suitable opportunities and ensure readiness.
• Track placements, outcomes, and referrals in the program data system with accuracy.
• Maintain contact with placed participants for 90 days post-employment with regular check-ins.
• Intervene early to support job retention and address workplace challenges.
• Assist patients with identifying interests, barriers, and readiness for work; aid resume and interview skills.
• Lead Job Club and skills groups to motivate participants and troubleshoot employment issues.
• Support applications for jobs, training, and certificates; assist with Certificates of Relief as needed.
