Champions Recovery is seeking an Enhanced Care Manager and Housing Navigator to support individuals with health barriers, housing instability, and community integration. The role emphasizes wrap-around services, care coordination, and collaboration with primary care and behavioral health providers.

Responsibilities include assessments, care planning, and linkage to housing resources, with outreach to shelters, landlords, and community organizations.

Qualifikationen

• BA in social work or related field; AA in social work or related field plus 2 years mental health experience; and/or 1 year of Housing Navigator experience may be substituted.
• Experience in mental health, homelessness, or related fields is required.
• Ability to work with diverse populations and trauma-informed care is preferred.

Aufgaben

• Provide wrap-around services using a person-centered approach to address client barriers.
• Coordinate care, health promotion, and connections to housing opportunities.
• Conduct Housing Assessments and develop Housing Support Plans.
• Engage in outreach to shelters, landlords, and community partners.
• Maintain documentation in electronic health records with timely entries.
• Attend community meetings and coordinate with providers and agencies.
