Job summary

CareLink Community Support Services is seeking a committed worker to promote recovery and wellness for individuals in our community-based programs. You will implement and document activities that support independence and self-determination, while managing resources and providing ongoing support to participants.

The role emphasizes advocacy, personalized service plans, and collaboration with families and community partners to enhance quality of life and community integration.

Qualifications

• High School Diploma or GED preferred.
• Bachelor's degree in a health/human services field preferred.
• Certifications, licenses or registrations may be required.

Responsibilities

• Assess needs and resources for individuals in service.
• Develop and implement service/recovery plans with client input.
• Advocate for individuals’ rights and promote self-determination.
• Conduct home visits and manage caseloads.
• Document progress notes, plans and incident reports.
• Coordinate community resources and transportation as needed.
• Support daily living activities, health and personal care.
• Ensure safety, emergency response, and program compliance.
