Job summary

HopeLink Behavioral Health seeks a Rehabilitation Specialist to support psychosocial day programs and community-based activities. You will develop client-centered recovery plans to help individuals regain skills and pursue goals while coordinating with case managers and healthcare partners.

The role involves psychoeducational services, crisis support, and documentation in line with policy and CARF standards; evenings/weekends may be required as needed to meet client needs.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field is required or preferred.
• QMHP-E certification required; QMHP certification preferred.
• Strong clinical written communication and presentation skills with ability to build rapport quickly with client and providers.

Responsibilities

• Develop Recovery/Transition Plans that empower clients toward goals.
• Provide rehabilitative skill-building in various settings including the day program and community.
• Deliver psychoeducation to improve health, living skills, and community integration.
• Support co-occurring disorders treatment and connections to services.
• Assist clients with volunteer placements, education, or consumer programs.
• Provide crisis support as needed and ensure proper documentation.
