Job summary

CareLink Community Support Services is seeking staff to provide activities of daily living support and recovery-oriented services for individuals with disabilities. You will document progress, assist with meals, housing cleanliness, and medication management while promoting independence and community integration.

The role emphasizes respectful, person-centered communication and involvement in daily therapeutic activities within a supportive team environment.

Qualifications

• Required Education: High School Diploma or GED
• Preferred Education: Bachelor's Degree
• Field of study: Human Services or related field
• Work Experience: 1 to 2 years
• Certifications, Licenses, Registrations: CPRP/RPRP preferred, Certified Peer Specialist preferred.

Responsibilities

• Develop and implement service plans reflecting the individual’s preferences, goals, and interests.
• Assess, teach, evaluate daily living skills and community activities.
• Document progress and staff interventions daily.
• Assist with medication regimen including PRN meds.
• Support recovery, wellness, and independence.
• Assist with maintaining clean and orderly living spaces and meals.
