Qualifikationen

• High school diploma or equivalent is required.
• Medi-Cal Peer Support Specialist Certificate preferred and pay differential offered.
• Lived experience recovering from a mental illness and/or substance use disorder or as a primary caregiver for someone with such experiences.
• Strong computer proficiency and ability to use electronic records systems with minimal direction.
• Desire to help others through recovery, with compassion and patience.

Aufgaben

• Maintain an active caseload of participants requiring intensive, community-based support.
• Build trusting relationships and model recovery principles such as self-determination and resilience.
• Collaborate with clinical staff and multidisciplinary team for coordinated care.
• Provide outreach in the community to housing resources, healthcare, and supports.
• Assist participants in accessing services and developing personalized recovery plans.
• Document services in accordance with agency, county, and program requirements.
