Job summary

Advantage Behavioral Health Systems seeks a CPS-certified Peer Case Manager to support clients discharging from the BHCC Crisis Center. You will engage with clients and families, provide peer support, and develop Wellness Recovery Action Plans in collaboration with a multi-disciplinary team.

You will work across the ten-county catchment area, following treatment plans, documenting care, and coordinating with providers to improve aftercare engagement and ongoing recovery outcomes.

Qualifications

• Candidates must have leadership, advocacy, or governance experience.
• Strong reading and written communication skills required.
• Familiarity with databases and spreadsheets; CPS certification preferred.

Responsibilities

• Meet with clients and families to address issues and treatment plan goals.
• Assist in Wellness Recovery Action Plan creation.
• Provide social and psychological support and discuss progress.
• Assist with activities of daily living as needed.
• Document sessions and communications per policy.
• Coordinate transportation and scheduling for medical appointments.
• Collaborate with service providers on planning changes.
• Track data for performance indicators.
