Advantage Behavioral Health Systems in Athens, GA is seeking a BHCC Peer Case Manager to provide outreach, engagement, peer support, and case management to clients discharging from the Crisis Center. The role requires the candidate to serve as a client advocate and model recovery and wellness, using personal recovery experience.

The position operates Monday–Friday, 8:30 am–5 pm, serving the ten-county Advantage catchment.

Qualificações

• Must be a certified peer specialist (CPS) to retain this position.
• GED or high school diploma; at least 18 years old.
• Strong reading comprehension and written communication skills.

Responsabilidades

• Provide outreach, engagement, peer support and case management to clients after discharge.
• Follow up with clients by phone and in person in the community.
• Document progress notes and referrals timely per agency policies.
• Assist clients with activities of daily living and transportation arrangements as needed.
• Collaborate with service providers and other staff to support recovery.
• Maintain confidentiality in line with policy.
• Track data for key performance indicators to demonstrate grant objectives.
