Job summary

The Commonwealth of Pennsylvania seeks a Drug and Alcohol Case Management Specialist Trainee to guide clients through assessments, service coordination and case management activities in support of recovery and community health.

Applicants must have a Bachelor's degree (or within 3 months of graduation) in a related field or an equivalent combination of education and experience, with in-person work in Pennsylvania and strict confidentiality adherence.

Qualifications

• Bachelor’s Degree in Chemical Dependency, Sociology, Social Welfare, Psychology, Nursing or related field or 18 college credits in related social sciences; or
• Any equivalent combination of experience and training.

Responsibilities

• Client Screening: Conduct telephone and face-to-face screening of clients during initial referral contact
• Service Assessment: Conduct assessments in office or community settings to identify service needs
• Care Determination: Determine appropriate level of care based on placement guidelines
• Plan Coordination: Coordinate access to services and update Recovery Plans as needed
• Authorization Processing: Request funding authorization and complete required documentation
• Documentation Management: Document case activities and ensure confidentiality compliance
