Job summary

Resources for Human Development (RHD) in Charlotte, NC is seeking a compassionate Case Manager for the Community Transitional Recovery Program (CTRP). The role focuses on guiding participants with complex behavioral health needs from institutional or crisis settings back into the community, helping them identify goals and access essential resources.

You will work within a multidisciplinary team to ensure recovery plans are implemented, barriers are overcome, and participants gain lasting

Qualifications

Bachelor's Degree in Human Services or related field.
At least 2 years of general work experience supporting individuals in behavioral health, addiction recovery, or IDD.
Valid Driver's License.
Ability to complete electronic documentation and provide transportation.

Responsibilities

Identify resources and supports needed to meet each participant's individualized needs.
Advocate, coordinate resources, and participate in a multidisciplinary treatment team.
Assist participants in transitioning to community living and maximizing independence.
Coordinate with team to develop and review individualized recovery plans.
Provide transportation to appointments and accompany participants as needed.
Document services accurately and within required timeframes.
