Resources for Human Development (RHD) is seeking a part-time Case Manager for the Community Transitional Recovery Program (CTRP) in Raleigh, NC. The role focuses on supporting individuals with high behavioral health needs as they transition from institutional settings into community-based living, coordinating resources, and promoting recovery and independence.

The Case Manager will collaborate with a multidisciplinary team, facilitate individualized recovery planning, and help participants

Qualifications

• Bachelor’s Degree required in human services or related field.
• At least 2 years of experience providing support to individuals in behavioral health/recovery.
• Valid driver’s license and ability to transport participants.
• Intermediate computer skills and ability to complete electronic documentation.
• Ability to provide community-based services and coordinate resources.

Responsibilities

• Identify resources and supports to meet each participant’s individualized needs.
• Advocate, coordinate resources, and work with a multidisciplinary team.
• Develop and review individualized recovery plans.
• Support participants in transitioning to community living and accessing services.
