Job summary

ICL is seeking a dedicated case manager to join a team delivering client-centered rehabilitation and recovery services for adults with mental illness. You will help clients develop skills to live, learn, work, and socialize in community settings of their choice, under supervision.

You will coordinate treatment plans, provide counseling, maintain records, and ensure services respect cultural and language needs while supporting discharge planning and community reintegration.

Qualifications

• Bachelor's degree in a human services field or equivalent.
• Experience providing direct services in mental health or similar setting.
• Familiarity with case management and service planning.

Responsibilities

• Secure information contributing to the individual's situation and maintain treatment plans.
• Provide supportive counseling in 1:1 or group formats to modify attitudes/behaviors.
• Maintain accurate case records and develop measurable service plans with quarterly reviews.
• Facilitate services to meet discharge goals and document significant events and contacts.
• Use cognitive-behavioral techniques to help clients meet service plan goals.
• Encourage adherence to medications and medical appointments to meet treatment goals.
• Monitor medications and maintain medication records per agency policy.
• Accompany clients to medical appointments and act as liaison with providers/ICL.
• Plan activities and obtain resources; advocate with outside service providers.
• Perform crisis assessment and nonverbal/verbal crisis intervention.
