Sandra Eskenazi Mental Health Center is seeking a Care Coordinator to deliver client-centered skills training and collaborate with the treatment team to assess needs and develop treatment plans. The role emphasizes advocacy and connecting clients to necessary services within a supportive community framework.

The position requires a Bachelor’s degree and Indiana LBSW licensure if applicable, with a valid Indiana driver's license. Full-time schedule; dayshift; on-site location in Indianapolis.

Formación

• Bachelor’s degree required.
• If holder has a Bachelor’s in Social Work, must possess an active LBSW in Indiana.
• Valid Indiana driver’s license and driving record must meet policy requirements.

Responsabilidades

• Adheres to Recovery Model and motivational interviewing principles.
• Provides skills training in group and individual settings as guided by treatment plans.
• Conducts client assessments and revises treatment plans with the treatment team.
• Manages crises and emergency interventions as needed.
• Documents timely and accurately per service and billing procedures.
• Provides case management with family and community supports to meet goals.
• Maintains a safe environment and supports continuity of care.
