Job summary

The Sandra Eskenazi Mental Health Center in Indianapolis is seeking a Grant-Funded Care Coordinator (Outreach & Engagement). You will provide skills training and development to clients, conduct assessments in collaboration with the treatment team, and help formulate and revise person-centered treatment plans.

Embracing the Recovery Model and AIDET, you will advocate for clients to access needed services, maintain documentation, and support community resources.

Qualifications

• Bachelor’s degree is required.
• If the degree is in social work, a current, unencumbered Indiana LBSW license is required.
• Valid Indiana driver’s license and driving record must meet policy requirements.

Responsibilities

• Provide skills training and development to clients in group and individual settings as guided by treatment plans.
• Collaborate with the treatment team to assess clients and revise treatment plans.
• Advocate for clients to access services and build community resources.
• Provide crisis management and emergency interventions in line with policy.
• Document and bill services accurately and timely according to guidelines.
• Transport clients using agency or personal vehicle as directed by the team.
