Qualifications

• High school diploma or GED required; associates degree or higher in a field related to behavioral health preferred.
• 1 year of experience in a behavioral health position providing outreach and engagement activities.
• Current AZ Driver's License and 39-month MVR.
• Behavioral Health Technician (BHT) per policy and procedures.
• Peer Certification (if applicable).
• Arizona Fingerprint Clearance card.
• Jail Clearance (if applicable).
• Recovery from Alcohol or Drugs and/or GMH for equal to or greater than 1 consecutive year preferred.

Responsibilities

• Provide community-based case management to patients experiencing serious mental illness or high acuity.
• Assist in planning, coordination, advocacy, and monitoring; help patients access health services, housing, employment, and social supports.
• Develop and monitor individualized treatment plans and document services provided in a timely manner.
• Promote a recovery-focused environment and implement advanced techniques for high-risk clients.
