Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide community and/or office-based services, coordinating care and supporting patients and families.

You will assist with access to health services, financial aid, employment, education, and social supports, while documenting progress and maintaining a recovery-focused environment. The role requires a high school diploma or GED (associate degree preferred), GMH/SU experience, and valid AZ credentials.

Qualifications

• Must have a Highschool diploma or GED; associate degree preferred.
• Recovery from substance use and/or mental health disorders: 6 months minimum preferred.
• 1–3 years of GMH/SU population experience required.
• 1–3 years in behavioral health with outreach/engagement activities required.
• 1–3 years in a medical setting preferred.
• Valid identification required.
• Arizona Driver's License preferred; 39-month MVR preferred.
• BHT certification per A.A.C. R9-10-101.33 and CBI policies required.
• Arizona Fingerprint Clearance card

Responsibilities

• Provide case management in community or office settings.
• Plan, connect, outreaching, advocate, coordinate and monitor patient needs and progress.
• Assist patients in gaining access to health services, financial assistance, employment, education, social services and natural supports.
• Assess patient needs and develop/monitor individualized treatment plans; document services timely.
• Promote recovery and a safe environment for clients.

Job Summary

The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.

Skills/Requirements

• Highschool diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
• Minimum of six month of recovery from substance use and/or mental health disorders preferred.
• 1-3 years of experience working with the GMH/SU population required.
• 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
• 1-3 years of experience in a medical setting preferred
• Valid identification required
• Current AZ Driver's License (valid and in good standing), preferred.
• 39-month MVR preferred
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
• Arizona Fingerprint Clearance card
