Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in community and/or office settings, helping patients access health services, financial aid, and social supports.

The role focuses on coordinating care, developing treatment plans, and documenting services accurately to support recovery for individuals facing crisis and behavioral health challenges. The Case Manager will engage with patients, families, and care teams, promoting a safe recovery environment

Qualifications

  • Highschool diploma or GED required; Associates Degree or higher in a field related to behavioral health preferred.
  • Minimum of six month of recovery from substance use and/or mental health disorders preferred.
  • 1-3 years of experience working with the GMH/SU population required.
  • 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
  • 1-3 years of experience in a medical setting preferred.
  • Valid identification required.
  • Current AZ Driver's License (valid and in good standing), preferred.
  • 39-month MVR preferred.
  • Behavioral Health Technician (BHT) per policy and credential requirements.
  • Arizona Fingerprint Clearance card.

  • Assess patient needs, develop and monitor individualized treatment plans, and document services timely.
  • Coordinate care with the care team, including outreach and engagement activities.
  • Embrace recovery philosophy and promote a safe recovery environment for clients.

Case Manager

Community Bridges, Inc. (CB I) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.

Job Summary

The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.
