Job summary

Highland Rivers Behavioral Health in Rome, GA seeks an ACT case manager to provide 24/7 community-based, multidisciplinary support within the ACT program. The role focuses on developing Individual Recovery Plans addressing medical, behavioral, social, housing and other needs to help clients progress toward recovery.

The position requires a 4-year degree and at least two years of direct care experience in human services, with bilingual English/Spanish preferred.

Qualifications

• Bachelor's degree required.
• Minimum of two years providing direct care to individuals in a human services field and electronic documentation.

Responsibilities

• Community-based position providing ACT services.
• Assist with activities of daily living and resource development.
• Link and advocate with other agencies to secure resources for individuals.
• Provide crisis support and ongoing assessment and monitoring.
• Document services and ensure eligibility criteria are met and records are accurate.
