Responsibilities

• Monitor client progress and engagement to identify early signs of potential treatment discontinuation.
• Conduct regular check-ins with clients to support engagement and transitions into the program.
• Build trust, rapport, and a strong sense of connection within the Isaiah House community.
• Provide empathetic support, active listening, and address client needs and concerns.
• Identify early warning signs of disengagement or distress and respond appropriately.
• Provide immediate support and de-escalation for clients at risk of leaving.
• Assist clients in problem-solving challenges related to treatment and personal stressors.
• Educate clients on the importance of continued treatment and recovery processes.
• Collaborate with clinical staff to address barriers impacting client commitment.
• Develop strategies to enhance client retention and motivation.
• Maintain accurate documentation of client interactions in line with policy.
• Participate in staff meetings and contribute insights to improve engagement.
• Reinforce Isaiah House's core values and mission in client care.
