Job summary

Isaiah House Treatment Center in Kentucky seeks a compassionate Targeted Case Manager to support clients on their recovery journey by connecting them with essential services and resources. You will assess needs, develop service plans, coordinate referrals, and monitor progress within a multidisciplinary team.

The ideal candidate will be organized, resourceful, and committed to helping clients achieve stability and independence.

Qualifications

Bachelor's degree required; one year of post-graduate professional experience preferred.
Experience in case management, behavioral health, or related field is preferred.
Employment contingent on successful pre-employment screening (drug screen, background check, Form I-9).

Responsibilities

Manage assigned caseloads with assessment, service planning, progress monitoring, and closure.
Assist clients in identifying goals supporting recovery, wellness, stability, independence, and self-sufficiency.
Coordinate referrals, community resources, appointments, and provider communications.
Advocate for clients to understand services and treatment options.
Maintain relationships with clinical staff, medical providers, and community agencies.
Maintain confidential, accurate case documentation and progress notes.
