Qualifications

• 3 years of full-time or equivalent experience in human services or social work.
• Ability to communicate effectively with diverse populations and stakeholders.
• Ability to maintain complete and up-to-date client records and to coordinate ISP processes.

Responsibilities

• Provide direct care to clients and help obtain services to support community living.
• Coordinate ISP with clients and service providers; convene meetings and monitor implementation.
• Chairs treatment team meetings and participates in discharge planning and reviews.
• Provide counseling and assist in developing independent living skills.
