FCC Behavioral Health in Ripley County, MO seeks a Care Coordinator to support individuals and families in managing illness, accessing resources, and achieving recovery in daily living, work, and community activities.

The role involves education, transportation support, and linking clients with community resources while maintaining documentation and adherence to agency standards and evidence-based practices.

Qualifications

• Master’s or bachelor’s degree in a human services field
• Qualified Addiction Professional (QAP) or
• Associate of Applied Science in Behavioral Health Support
• Two years of qualifying experience; four years acceptable for alternative qualification

Responsabilités

• Provide education to person(s)-served about mental illness and/or substance use disorder.
• Provide transportation for person(s)-served when medically necessary.
• Link person(s)-served with community resources and natural supports.
• Help person(s)-served develop interpersonal, social, and independent living skills.
• Complete required documentation in a timely manner.
• Adhere to evidence-based practice models.
• Demonstrate knowledge of diagnostic criteria for substance use and/or mental disorders.
• Communicate information effectively and accurately.
• Ensure program operates within agency standards and strategic plan.
• Abide by program and agency policies and procedures.
• Perform other duties and special projects as assigned.
