Qualifications

Responsabilités

• Collaborate with members to create individualized recovery plans.
• Empower members to take ownership of their recovery plans.
• Review recovery plans every 90 days and make necessary revisions.
• Connect members with recovery resources and support.
• Complete and submit accurate progress notes and documents.
