Job summary

Qualifications

Complete required training and obtain PSS Certification.
Engage in PSS meetings and trainings.
Demonstrate effective interpersonal skills and cultural sensitivity.
Provide follow-up contacts to ensure continuity of care.

Responsibilities

Provide peer support services across LOC-based settings, including individual and group sessions.
Document services using collaborative documentation systems.
Assist with housing, food, and other social needs navigation.
Maintain compliance with agency policies and training requirements.
Participate in team meetings and continuous education.
Support recovery and continuity of care for patients served.
