Job summary

You will identify service needs, engage patients with motivational interviewing, coordinate discharge and warm handoffs, and document care in the EMR, ensuring compassionate care that respects culture and dignity.

Qualifications

• CPRS certification within 2 years (IAODAPCA) is required.
• Illinois SOS driver's license required.
• Proof of auto insurance required.

Responsibilities

• Identify patient needs and enroll them into services with warm handoff.
• Recruit and engage patients using motivational interviewing to connect to MAT.
• Engage patients with SUD and facilitate warm handoffs between care teams.
• Establish collaborative relationships and provide supportive treatment planning.
• Assist in creating and documenting treatment and discharge planning.
• Ensure patient contact respects dignity and cultural beliefs.
• Participate in interdisciplinary staffing and share pertinent information.
• Demonstrate motivational interviewing skills with patients.
• May transport patients to appointments.
• Provide continuum of care and follow-up after discharge.
• Educate patients and families about addiction, naloxone, and treatment coverage.
