CVS Health is seeking a Peer Support Specialist in Virginia to guide Aetna members recovering from behavioral health and/or substance use challenges. The role relies on lived experience to coach members, coordinate care, and share coping strategies, wellness tools, and resources.

Responsibilities include coordinating with care teams, educating others on recovery-based approaches, and helping address social determinants of health to reduce hospitalizations and recidivism.

Qualifications

• Experience working with individuals in recovery and justice-involved settings.
• Excellent communication and collaboration abilities.
• Comfort working in diverse populations and coordinating with multiple teams.

Responsabilités

• Attend case rounds and assist in coordinating member care.
• Educate care management team on recovery-focused support.
• Collaborate with health providers and justice system staff on recovery principles.
• Identify social determinants affecting health and coordinate plans.
• Attend medical or behavioral health appointments when appropriate to support adherence.
• Assist members in setting short- and long-term recovery goals.
• Encourage participation in wellness activities at a comfortable pace.
