Job summary

CVS Health is seeking a Peer Support Specialist to guide and mentor members through personalized recovery journeys in Louisiana, using lived experience to support coordinated care. Qualifications

• Must reside in Louisiana, specifically in the SE portion of the state (i.e. Baton Rouge, Northshore, New Orleans).
• Ability to travel 50% of the time to multiple office and home locations with personal vehicle, reliable transportation, valid driver’s license, and automobile insurance is required. CVS reimburses for mileage.
• 1-2 years’ experience in community/outpatient setting working with members in a mental health or substance use setting is required.
• 1-2 years experience providing care to socially and medically complex patients effectively in a variety of non-traditional settings.
• 2-3 years of knowledge of formal and informal community agencies and resources. Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.

Responsibilities

• Attend case rounds and assist the team in identifying members who may benefit from peer support services.
• Educate care management teams on how peer support can help members recover and coordinate care.
• Collaborate with health professionals on recovery principles and social determinants of health.
• Assist in helping members identify socio-economic issues affecting health and create person-centered plans.
• Attend medical or behavioral health appointments with members to support connections to community resources.
• Conduct face-to-face and virtual visits to engage members in goal setting.
• Help members set short- and long-term recovery goals.
• Suggest approaches to encourage participation in wellness activities.
• Support members in developing their voice and choice in care processes.
• Advocate for members and ensure goals align with expressed needs.
