Job summary

Horizon House is seeking a compassionate Recovery Coach to support individuals in achieving recovery, independence, and community integration. You will coordinate services, assist with treatment planning, and connect participants to essential community resources within a multidisciplinary team.

The role emphasizes person-centered care, collaboration, and ongoing advocacy, with local travel to support community-based work and emergencies as needed.

Qualifications

• Demonstrated ability to work independently and exercise sound professional judgment.
• Experience monitoring participant progress and maintaining service documentation.
• Strong verbal and written communication skills.
• Commitment to a recovery-oriented, person-centered philosophy of care.
• Valid driver's license with an acceptable driving record.

Responsibilities

• Serve as the primary coordinator for participants' treatment, recovery, and community integration plans.
• Partner with participants to identify goals, develop action plans, and support progress toward greater independence.
• Provide ongoing case management and recovery support services to participants and their families.
• Connect participants to community resources, healthcare providers, employment services, housing supports, and other essential services.
• Monitor participant progress and evaluate effectiveness of services and supports.
• Maintain accurate, timely, and compliant documentation of participant interactions and service delivery.
• Collaborate with internal teams and external providers to ensure coordinated, holistic care.
• Participate in treatment planning and interdisciplinary team meetings.
• Facilitate PRS groups.
