Job summary

Catholic Charities in Cottonwood, AZ, is seeking a Recovery Coach/Substance Abuse Case Coordinator to mentor and support clients through recovery. This role emphasizes outreach, education, and individualized service planning within the AFF program.

You will document services, coordinate assessments, and collaborate with an interdisciplinary team, requiring a background in human services, strong communication, and reliable transportation.

Qualifications

• Requires Associate’s degree in human services or behavioral health or equivalent experience.
• Experience with behavioral health services, child welfare, or substance abuse is required.
• Must be substance-free for at least six months post-treatment.

Responsibilities

• Provide initial 24‑hour in-person outreach and follow up.
• Deliver recovery education for every phase of the recovery journey.
• Assist clients to identify goals and develop personal recovery plans.
• Promote self-advocacy and client voice in rehabilitation efforts.
• Coordinate services, including advocacy, education and resources.
• Maintain accurate documentation and monthly reporting.
