Responsibilities

  • Assist clients to identify goals and develop personal recovery plans.
  • Promote self-advocacy and client voice in rehabilitation efforts.
  • Coordinate services, including advocacy, education and resources.
  • Maintain accurate documentation and monthly reporting.

JOB SUMMARY: Serves as a role model, mentor, advocate and motivator to recovering individuals in order to help prevent relapse and promote long-term recovery. Provides outreach and support as needed. Addresses affects of substance abuse with clients and their families while providing comprehensive case management and education throughout their participation in the AFF program.

JOB DUTIES:

  • Provides initial 24 hour in person outreach and follow up to families referred by Department of Child Safety (DCS).
  • Provides recovery education to service recipients for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance.
  • Be a model by demonstrating that recovery is possible.
  • Assist recovering persons to identify their personal interests, goals, strengths and weaknesses regarding recovery.
  • Assist and coach recovering clients to develop their own plan for advancing their recovery.
  • Promote self‑advocacy by assisting recovering clients to have their voices heard; their needs, goals and objectives established as the focal point of rehabilitation and clinical services.
  • Participates in interdisciplinary team meetings, as needed.
  • Oversees the collection and review of all collateral information including but not limited to: social histories, psychological evaluations, judicial records, previous substance abuse treatment information, DPS reports, and obtaining a release of information for everyone involved in the case.
  • Provide on‑going support, education and case management expertise to those experiencing substance use.
  • Work with clients to develop individualized service plans and update quarterly or more frequently as needed.
  • Submits referral for Substance Abuse Assessment and assists in coordination of Adult Recovery Team meeting.
  • Coordinates all services for each individual client, including advocacy, education while adhering to curriculum fidelity as prescribed, and providing information about community resources, etc.
  • Provides accurate, timely and detailed documentation of services. Completes monthly progress reports and billing as required and records in Agency and DCS software systems.
  • Promote ongoing commitment to excellence and continual quality improvement to ensure the highest quality of services to our staff and clients.
  • Performs other duties as assigned.

JOB REQUIREMENTS:

  • Education /Background : A minimum of an Associate’s degree in a human service or behavioral health‑related area of study, or a High School Diploma or equivalent with at least three years of related experience including working with multi‑problem families in the field of substance abuse, child welfare, or behavioral health. Must have been a behavioral health service recipient or substance abuse recipient. Must be substance‑free for a minimum of six months following treatment. Must have been a behavioral health service recipient or substance use treatment recipient and completed identified goals and objectives. Must be a paraprofessional with training (or be able to obtain paraprofessional certification within 90 days of employment), and/or received a certification in a substance use disorder or Recovery Coach Program.
  • Job Knowledge : Must have knowledge and understanding of human behavior, social work theories and techniques as well as community resources. Must have counseling, interviewing assessment, therapeutic relationship skills. Must be flexible and able to work with the uniqueness of each client. Must be sensitive to cultural differences within the agency and community. Fluent in Microsoft Office Applications (Word & Outlook).
  • Working Conditions/Physical Demands : Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps. Must be able to visually and auditory assess and interact with clients to ensure their safety and wellbeing. Requires considerable work outside of the office and substantial driving; requires evening and weekend work on occasion and willingness to be on‑call for emergencies; may lift up to 25 pounds.
  • Other requirements
    • Valid AZ driver’s license, reliable transportation and ability to show proof of registration/insurance.
    • Level One Fingerprint clearance or ability to obtain fingerprint clearance; must maintain clearance.
    • You must be 21 years of age or older to drive on behalf of Catholic Charities.

Hourly rate starting at $19.11 may increase depending on experience.
