Qualifications

• Must be at least 21 years old.
• Good driving record; valid AZ driver’s license and auto insurance.
• Reliable transportation and proof of vehicle insurance.
• Proof of physical exam and annual TB skin test.
• Fingerprint clearance if working with children or on premises with children’s services.

Responsibilities

• Provides services to adults, children, and families; assists in developing, evaluating, and coordinating treatment plans and recovery services.
• Works with clients to implement recovery plans and coordinate services across provider networks.
