Job summary

Southwestern Behavioral Healthcare seeks a full-time Addiction Services Case Manager to support adults with chronic substance use histories. The role focuses on coordinating transitional residential services, providing case management and skills training, and supporting clients throughout recovery.

You will evaluate admissions, coordinate care with the treatment team, maintain clinical documentation, and build community partnerships to ensure high-quality, ongoing client support.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice, or related field.
• Experience with adults with a substance use disorder preferred.
• Valid driver’s license and eligibility for company vehicle liability insurance.
• Must pass background checks including county/state checks, CPS check, sex offender registry check, and drug screen.

Responsibilities

• Evaluate and screen applicants for admission to the program.
• Coordinate admissions to the service, as well as discharges from the program.
• Monitor living environment, involvement in community living and self-help groups, employment, and payment of fees.
• Provide skills training and care coordination to Transitional Residential patients.
• Develop appropriate treatment plans and goals and monitor patient’s progress toward treatment goals.
• Coordinate patient care with other members of the treatment team.
• Maintain contact with referral sources and other providers as necessary.
• Coordinate reentry into the community.
• Maintain appropriate clinical documentation.
