Job summary

Southwestern Behavioral Healthcare is seeking a full-time Addiction Services Case Manager to coordinate transitional residential services, provide case management and skills training, and support adults with chronic substance use histories on their recovery journey in Evansville, IN.

The role requires a Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or related field; experience with adults with a substance use disorder is preferred.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Criminal Justice or related field.
• Experience with adults with a substance use disorder is preferable.

Responsibilities

• Evaluate and screen applicants for admission to the program.
• Coordinate admissions and discharges from the program.
• Monitor living environment and community involvement, including self-help groups and employment.
• Provide skills training and care coordination to Transitional Residential patients.
• Develop treatment plans and monitor progress toward goals.
• Coordinate patient care with the treatment team.
