Job summary

Arundel Lodge is seeking a Case Manager to support residents in a Residential Rehabilitation Program. You will coordinate entitlements, link to community resources, assist with discharge planning, and promote independence through person-centered care.

You will advocate for clients across systems of care, provide transportation when needed, and document all activities in compliance with policies. A Bachelor’s degree in a related field is preferred, with at least one year of relevant experience.

Qualifications

• Bachelor’s degree preferred in Human Services, Psychology, Social Work, Rehabilitation, or related field.
• Equivalent experience may be substituted at the discretion of the Residential Director.
• Minimum one year of experience in mental health, residential, or psychosocial rehabilitation settings required.
• Experience with discharge planning, community linkage, and entitlement programs strongly preferred.

Responsibilities

• Conduct intake assessments and determine program eligibility and service needs.
• Complete and maintain benefit entitlement applications and renewals (SSI, SSDI, Medicaid, SNAP).
• Verify Representative Payee status and track residential fees with the business office.
• Assist clients with health insurance enrollment and re-certification.
• Participate in interdisciplinary team meetings and contribute to IRPs.
• Link residents to community resources like therapy, employment, education, and primary care.
• Advocate for clients to ensure access to services and fair treatment.
• Provide transportation to appointments as needed.
• Maintain documentation per agency policies and funding requirements.
• Collaborate with teams to monitor progress and revise service plans.
