Qualifications

• Bachelor’s degree preferred; equivalent experience may be substituted.
• Minimum one year of experience in mental health, residential, or psychosocial rehabilitation settings required.
• Experience with discharge planning, community linkage, and entitlement programs strongly preferred.
• Valid driver’s license with acceptable driving record; background check and drug screening required.

Responsibilities

• Conduct intake assessments to determine eligibility and service needs.
• Assist with entitlement applications (SSI/SSDI/Medicaid/SNAP) and renewals.
• Coordinate with treatment teams and link residents to community resources.
• Lead discharge planning and coordinate placement for higher levels of care when needed.
• Provide transportation to appointments and support with money management.
• Maintain documentation in accordance with agency policies and funding requirements.
