Job summary

Fairview Hospital / Cleveland Clinic in Knoxville seeks a Transition Recovery Home Case Manager to provide person-centered case management for adults in early recovery within a community-based residential setting.

You will develop and monitor individualized recovery plans, coordinate mental health, addiction, medical, housing, and social services, and document services in electronic health records while practicing trauma-informed, recovery-oriented care.
