Job summary

BriteLife Recovery in Hilton Head Island, SC is seeking a Case Manager to provide confidential addiction guidance, education and support to individuals, families, and the community, coordinating care and facilitating post-treatment placement.

Under the supervision of the Director of Clinical Services, you will work with a multidisciplinary team to help clients re‑enter society and engage in housing, employment and outpatient treatment as needed.

Qualifications

• Bachelor’s degree in behavioral health or related field is preferred.
• Certified Recovery Specialist or Certified Family Recovery Specialist preferred.
• Minimum two years of experience in substance abuse treatment.
• Demonstrated computer proficiency.
• Prior experience working in a healthcare setting preferred.

Responsibilities

• Develop discharge/transfer plans to ensure movement to appropriate care levels.
• Coordinate with referral sources for case management and referrals.
• Coordinate ongoing housing and therapeutic needs with the team and providers.
• Document all case management notes in the client’s electronic records.
• Promote BriteLife Recovery to clients, employees and referral sources.
• Obtain prior records relevant to the current treatment episode.
• Develop client-centered aftercare plans with follow-up information.
• Provide active case consultation with the multidisciplinary team.
• Attend supervision with the supervisor to discuss growth and concerns.
• Establish therapeutic boundaries and utilize clinical resources.
• Assist in monitoring therapeutic activities and support the team.
• Work directly with clients to provide recovery support.
• Participate in special projects as needed.
• Promote Sunspire Hilton Head to clients and employees.
