Job summary

Zinnia Health is seeking a dedicated Case Manager to coordinate patient care, document progress, and collaborate with the clinical team to deliver quality treatment services. The role emphasizes familiarity with 12-Step Recovery programs and documentation in an electronic medical record system.

You will work with the Primary Counselor and Clinical Director to ensure safe, patient-centered care and effective discharge planning, while navigating insurance coverage and potential financial

Qualifications

• High School Diploma or equivalent.
• Experience as group facilitator in substance use/mental health settings preferred.
• Familiar with 12 Step Recovery programs and EMR systems.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
• Handle case assignments, draft service plans, review case progress and determine case closure.
• Communicate patient needs to care team and follow up accordingly.
• Manage discharge plans upon completion of treatment.
• Attend clinical and administrative meetings for information sharing.
