Job summary

Recovery Centers of America (RCA) is seeking a Case Manager to develop and coordinate individualized aftercare plans, ensuring patients access needed resources after discharge. You will coordinate with clinical teams, families, and external providers to support transitions from inpatient treatment to community-based care.

Ideal candidates bring experience in case management and substance use disorder treatment, with strong communication and problem-solving abilities.

Qualifications

• High school diploma or GED; bachelor’s degree preferred.
• 1+ year in behavioral health/addiction treatment.
• Knowledge of community resources and recovery supports.
• Strong problem-solving and communication skills.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Develop and coordinate personalized aftercare plans, ensuring patients have access to resources after discharge.
• Assist patients in scheduling follow-up appointments, including primary care and mental health services.
• Advocate for patients, addressing housing, employment, legal concerns, and social support.
• Work closely with the clinical team to align aftercare plans with treatment goals and ensure continuity of care.
• Engage with families and support systems, providing education and resources.
• Maintain thorough documentation of discharge planning, referrals, and patient progress.
